Risk assessment form

A risk assessment form should be completed and returned to the CSO in advance of any trips or special events; please contact the CSO if you are not sure whether a risk assessment is necessary.

Trip registration form

A trip registration form, which includes details of all persons travelling, must be completed and returned to reception in advance of any trips.

Ffurflen asesu risg

Dylid cwblhau ffurflen asesu risg a’i dychwelyd i’r SCC cyn unrhyw wibdeithiau neu ddigwyddiadau arbennig; cysylltwch â’r SCC os gwelwch yn dda os nad ydych yn siŵr os oes angen asesiad risg.

Mae’n rhaid cwblhau ffurflen cofrestru taith, sy’n cynnwys manylion am bob person sy’n teithio, a’i dychwelyd at y SCC cyn cychwyn ar unrhyw daith.
